Community Health Assessment 2023
We are proud to officially release the 2023 Clay County Community Health Assessment (CHA) in collaboration with the Florida Department of Health in Clay County on September 27, 2023. This extensive report examines the county’s key health concerns, providing crucial data to inform future health initiatives.
With input from community leaders, residents, and key stakeholders, the assessment identified the following three major health priorities:
- Behavioral Health – mental health, substance use/abuse, smoking/vaping, domestic violence and child abuse
- Lifestyle Behaviors – obesity/overweight, smoking/vaping, chronic diseases, communicable diseases, access to healthy foods, health education and knowledge
- Healthcare Access – primary care, specialty care, and transportation options
These findings will guide targeted interventions and policy efforts to improve health outcomes for Clay County residents.

Community Health Improvement Plan 2024
Following the findings of the CHA, Clay County began collaborating on the Community Health Improvement Plan (CHIP) in November 2023 to develop targeted strategies for addressing key health priorities. Using a balanced scorecard approach, the CHIP sets clear goals, objectives, and measurable targets to drive meaningful progress.
The CHIP Action Plan aligns with national Healthy People 2030 goals and Florida’s State Health Improvement Plan (SHIP), ensuring evidence-based strategies for improving community health. Workgroups will continue collaborating to implement and evaluate initiatives focused on the three health priorities listed above.
