What are CHAs and CHNAs?

HPCNEF partners with county health departments and other organizations to conduct Community Health Assessments (CHAs) and Community Health Needs Assessments (CHNAs). These assessments are done every 3–5 years and require survey input from county residents on healthcare, quality of life, and other services.

Why should you participate in community health assessments?

  • Identify Health Issues: Help find out what health problems matter most in your community.
  • Shape Local Services: Make sure the health services offered match your community’s needs.
  • Promote Fairness: Ensure everyone’s voice is heard, especially those often overlooked.
  • Improve Health: Your input can lead to better health programs and healthier neighborhoods.
  • Keep Leaders Accountable: Make sure local leaders listen and respond to community concerns.
  • Get More Funding: Accurate information helps bring in money for better health services.
  • Build Community Strength: Working together helps create a healthier, stronger community.

Your voice can make a real difference!
