We enjoy long-standing professional partnerships throughout Northeast Florida. And we’re proud to provide Health Planning services to local health departments, hospitals, nursing homes, behavioral health facilities, health and social service non-profits, city and county government agencies, and colleges and universities.

Healthy communities consider regulatory policies that promote access to healthy built, social, economic, and natural environments. Program Management

Stakeholder workgroups meet to explore the gaps revealed by local data and identify projects to fund. We leverage our resources and experience to guide these projects. We have decades of partnership with various entities throughout Northeast Florida and the state, including non-profit organizations, for-profit corporations, governmental agencies, and educational institutes. As a publicly authorized yet autonomous non-profit corporation, HPCNEF is a strong bridge between public health and the private healthcare industry – offering an ideal platform to foster collaboration.

Systematic processes to gather, analyze, and report program or intervention outcomes

Granting agencies require measurements and outcomes of the funds invested in local communities, programs and projects. By assessing the quality or impact of funded programs, evaluation can help plan and implement new programs, make future grant decisions, and demonstrate accountability to the public trust.

Health Education

As subject matter experts, we assist organizations in various sectors across Northeast Florida by designing and presenting education modules. These modules raise awareness of current health trends and evidence-based best practices.
