County Health Profiles are published annually for our seven-county region which represents 2,406,668 people, or approximately 10.6% of Florida’s total population. The profiles provide a snapshot of the health status of each county in comparison to the state of Florida. These profiles include data for a range of categories:

  • Demographic data, including population age, race, ethnicity, and gender
  • Socioeconomic data including income, poverty, unemployment, and education
  • County Health Rankings
  • Disease and death rates including leading causes of death and infectious disease rates
  • Maternal and child health data including birth rates, birth outcomes, prenatal care, and childhood immunizations
  • Healthcare access information including insurance coverage and healthcare facility information
  • Healthcare utilization data including leading causes/diagnoses for emergency department and inpatient hospital visits
County Health Assessments (CHAs) and County Health Improvement Plans (CHIPs)

The health improvement planning process begins at the county level with County Health Assessments (CHAs). These assessments identify priority community health and quality of life issues within the county. The results of CHAs then guide County Health Improvement Plans (CHIPs). CHIPs outline long-term efforts to address the health priorities identified in CHAs and improve the overall health of the county. CHAs and CHIPs are a few of the products that we deliver at HPCNEF.
