The Health Planning Council of Northeast Florida is excited to announce the publication of a new type of County Health Profile: the Northeast Florida County Cancer Health Profiles (NFCCHP)!
What are these profiles?
The NFCCHPs are seven county-specific health profiles that highlight local and regional cancer data, social determinants of health (SDOH), and access to care in a visual and easily understood format. Each profile contains the latest data on the priority cancer types listed in the Florida Cancer Plan 2020–2025, such as breast cancer, prostate cancer, and colorectal cancer. Sources include the American Cancer Society, the Centers for Disease Control and Prevention, the Florida Department of Health, the U.S. Census Bureau, and peer-reviewed journals.
Who are these profiles intended for?
These profiles are for organizations that plan, fund, and deliver cancer prevention, screening, treatment, and psychosocial support in our Northeast Florida region of Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia counties. These include, but are not limited to, public health departments, hospital systems, cancer centers, Federally Qualified Health Centers (FQHCs), safety-net clinics, community health worker (CHW)/patient navigation programs, behavioral health programs, cancer survivorship programs, nonprofits, community-based organizations (CBOs), faith-based partners, and local coalitions addressing cancer-related social determinants of health (SDOH) and disparities. Grant writers, decision-makers, and the general public of Northeast Florida can also benefit from these profiles.
How do I use these profiles?
We designed these profiles especially for community-based organizations to apply these findings to inform outreach priorities and bolster future funding proposals, ensuring that resources are directed toward the populations of greatest need. These profiles are intended to equip stakeholders to quickly identify gaps in strategic planning and use data to demonstrate the cancer burden in our region.

What is the goal of these profiles?
By streamlining the data research process for community-based organizations seeking cancer control funding, we hope these NFCCHPs equip local organizations with the data they need to attract additional funding and services for underserved communities in Northeast Florida.
How are these profiles funded?
In October 2025, the Health Planning Council of Northeast Florida was invited to submit a proposal for the 2025–2026 Community Implementation Grant from Sylvester Comprehensive Cancer Center’s Office of Outreach and Engagement, Moffitt Cancer Center’s Office of Community Outreach and Engagement, University of Florida Health Cancer Center (UFHCC)’s Office of Community Outreach and Engagement, and Mayo Clinic Comprehensive Cancer Center’s Community Outreach and Engagement.
