Starting Point Behavioral Healthcare (SPBH) is partnering with the Health Planning Council of Northeast Florida and the St. Johns River Rural Health Network to conduct a Community Needs Assessment (CNA) as part of the certification process to become a Certified Community Behavioral Health Clinic (CCBHC). We invite all adults (18+) living or working in Nassau County and surrounding areas to participate in a brief 10-minute survey to share your thoughts on mental health, substance use, and the overall quality of life in our community.
Your input, along with insights from interviews and focus groups with key stakeholders, will help identify the most pressing needs in our community and inform efforts to improve mental health and substance use services. The findings from this CNA will be compiled into a report, helping SPBH better meet the needs of our community residents.
How You Can Help:
- Complete the Survey: Available in both English and Spanish, the survey can be completed online or via a printable version.
- Share with Your Network: Please share the survey with your friends, family, coworkers, and neighbors to help us ensure broad community participation.
Important Information:
- The survey is completely anonymous and while some demographic questions are included, all responses will remain confidential.
- The survey is open until December 3, 2024.
