We are pleased to officially publish the 2025 Flagler and Volusia Counties Community Health Improvement Plan (CHIP) in collaboration with the Florida Department of Health in Flagler County and the Florida Department of Health in Volusia County on July 31, 2025.
Following the findings of the 2024 Community Health Needs Assessment (CHNA), Flagler and Volusia county stakeholders began collaborating on the Community Health Improvement Plan (CHIP) to develop targeted strategies for addressing key health priorities:
- Economic Stability & Social and Community Context – income, poverty level, access to affordable housing, access to affordable healthcare, demographic-specific challenges (e.g., seniors aged 65+)
- Health Care Access and Quality – provider shortages, cost of care, lack of awareness of available services and proper health education
- Mental Health and Disorders & Drug and Alcohol Use – mental health, substance use/abuse, child abuse/neglect, domestic violence, suicide
The CHIP Action Plan aligns with national Healthy People 2030 goals and Florida’s State Health Improvement Plan (SHIP), ensuring the use of proven, evidence-based approaches. Workgroups will continue to work together to implement and evaluate initiatives focused on the identified health priorities.
