The Florida Department of Health in Clay County has launched its community survey today. Clay County residents are encouraged to fill out a 10-minute survey to share their opinions about healthcare and the quality of life in their county. The Clay Department of Health relies on this feedback to create a detailed County Health Improvement Plan (CHIP).
Clay residents are encouraged to share the survey with friends, family, coworkers, and neighbors. It is available in both English and Spanish. The survey deadline has been extended to Friday, July 7, 2023.
