Example map: Choropleth map with point data layer showing the percentage of people without health insurance in St. Johns County. Areas with a darker green color have a higher percentage of uninsured individuals. Health facility locations are represented by different symbols. Created with PolicyMap, January 2026.
When you can see the story your data tells, you can plan smarter, act faster, and make a stronger impact.

GIS maps are powerful tools for:
We’ve supported many organizations with maps for diverse topics and datasets.

Example map: Choropleth map with point data layer showing the percentage of people with depression in Duval County. Areas with a darker purple color have a higher percentage of people living with depression. Various mental health treatment facilities are represented by different symbols. An Initial Request Meeting with a Health Planner to discuss your mapping goals, desired data indicators, branding preferences (colors, logos), and technical needs.
You will receive a Data Indicator Checklist to select data for your map. Most maps include 1–3 indicators. Additional maps may be needed for more indicators.
The Health Planner will review your Data Indicator Checklist and provide an estimate of the number of maps needed and the expected timeline.
You will receive a draft map for review. Each map includes up to 2 rounds of revisions. Revision meetings can be scheduled to review changes and feedback.
You will have direct support from a Health Planner throughout the project, including communication, guidance, and answers to technical questions.
Your completed map will be delivered in your chosen file format. You will also receive a written explanation of the map, perfect for describing your map in reports and presentations.
A video-recorded explanation by a Health Planner may be available for an additional fee.
Our Health Planning Team—trained in public health and skilled in data storytelling—creates GIS maps that visually represent your data in meaningful ways. Using Esri ArcGIS and PolicyMap, we design maps that reveal patterns, trends, and relationships that raw numbers alone can’t show.
Our team’s qualifications include:
We’ve successfully used GIS technology in several initiatives, including Advancing Health Literacy (AHL) and The Promote, Protect & Improve (PPI) Program in Duval County, to help communities better understand health trends and local needs.
As your county’s Local Health Council, HPCNEF operates as a non-competitive (sole source) contractor. Local governments and community partners can work with us directly without the long, complex process of competitive bidding. This ensures a smoother contracting experience and quicker access to our support.
Contact HPCNEF by emailing bi****@****ef.org. Let us know what type of data you are looking for in your GIS map. We will contact you to schedule an Initial Request Meeting with a Health Planner.
