Our ServicesCommunity Insight Maps

Visualize need. Strategize impact.

Geographic Information Systems (GIS) is a tool that allows us to create, manage, analyze, and visualize many types of data. It links information to geographic locations, bringing data and mapping together. A variety of GIS products can be created to visualize what’s happening in your community, including map images and interactive maps embedded on a website. GIS maps are important tools with many applications: Community Health Assessments, data reports, program planning, grant proposals, and presentations.
St. Johns County Uninsured Population & Health Facilities

Example map: Choropleth map with point data layer showing the percentage of people without health insurance in St. Johns County. Areas with a darker green color have a higher percentage of uninsured individuals. Health facility locations are represented by different symbols. Created with PolicyMap, January 2026.

Why GIS Matters

“The visualization of public health data serves as an essential tool to support understanding and conclusion-based decision making for policymakers along with public health professionals as well as other professions.”
— Ofori et al.

When you can see the story your data tells, you can plan smarter, act faster, and make a stronger impact.

What You Receive When You Order a Map

GIS maps are offered as a $600 per-map subscription service and include:
Initial Request Meeting

An Initial Request Meeting with a Health Planner to discuss your mapping goals, desired data indicators, branding preferences (colors, logos), and technical needs.

You will receive a Data Indicator Checklist to select data for your map. Most maps include 1–3 indicators. Additional maps may be needed for more indicators.

Data Indicator Selection and Map Estimate

The Health Planner will review your Data Indicator Checklist and provide an estimate of the number of maps needed and the expected timeline.

Draft Maps and Revisions

You will receive a draft map for review. Each map includes up to 2 rounds of revisions. Revision meetings can be scheduled to review changes and feedback.

Technical Assistance

You will have direct support from a Health Planner throughout the project, including communication, guidance, and answers to technical questions.

Final Map and Explanation

Your completed map will be delivered in your chosen file format. You will also receive a written explanation of the map, perfect for describing your map in reports and presentations.

A video-recorded explanation by a Health Planner may be available for an additional fee.
