SITUATION: Under the Strengthening U.S. Public Health Infrastructure, Workforce, and Data Systems Grant, federal funding was invested in core infrastructure improvements for public health agencies. A portion of the Public Health Infrastructure Grant (PHIG) has been awarded to the Florida Department of Health in Duval County (DOH-Duval) from the Centers for Disease Control and Prevention (CDC), which will assist in supporting and sustaining the Department’s workforce and in foundational public health services to target individuals that are disproportionately at risk for experiencing negative health outcomes.

PROGRAM OBJECTIVE: HPCNEF has partnered with DOH-Duval to provide contract management and monitoring services on this multi-year CDC grant, which addresses the areas of maternal and child health, food insecurity, and employee wellness. These public health priorities were identified in DOH-Duval’s Strategic Plan and Community Health Improvement Plan (CHIP).

IMPACT: 

  • Provides oversight of program development and implementation with multiple subcontractors
  • Oversees fiscal processes to streamline collaboration between partners and DOH-Duval, including invoicing and reimbursement activities
  • Conducts regular programmatic evaluation of partners
  • Provides technical assistance with contractual terms
  • Collaborates with partners and DOH-Duval to address challenges and ensure the successful completion of deliverables
  • Collects and analyzes aggregate data from subcontractors
