We are pleased to officially publish the 2024 St. Johns Community Health Improvement Plan (CHIP) in collaboration with the Florida Department of Health in St. Johns County on January 24, 2025.
Following the findings of the 2024 Community Health Assessment (CHA), St. Johns County began collaborating on the Community Health Improvement Plan (CHIP) to develop targeted strategies for addressing key health priorities. Using a balanced scorecard approach, the CHIP sets clear goals, objectives, and measurable targets to drive meaningful progress from 2025 to 2030.
The CHIP Action Plan aligns with national Healthy People 2030 goals and Florida’s State Health Improvement Plan (SHIP), ensuring the use of proven, evidence-based approaches. Workgroups will continue to work together to implement and evaluate initiatives focused on the identified health priorities.
