St. Johns River Rural Health Network (SJRRHN)

SITUATION: A significant gap in health exists between rural and urban citizens. Rural citizens are more likely to die from heart disease, cancer, unintentional injury, chronic lower respiratory disease, and stroke than their urban counterparts. These health disparities that rural populations experience are influenced by factors like geographic isolation, lower socioeconomic status, higher rates of health risk behaviors, and healthcare workforce shortages.

PROGRAM OBJECTIVE: HPCNEF partners with SJRRHN to “ensure that quality health care is available and efficiently delivered to all persons in rural areas” within our service area.

SERVICE AREA: Baker, Clay, Flagler, Nassau, Putnam, St. Johns, and Volusia counties

IMPACT:

  • Providing technical assistance to organizations serving rural areas to remove barriers to health care
  • Organizing outreach and education to inform the public about rural health issues
  • Conducting studies, surveys, planning, and development activities to analyze regional health care issues and deploy relevant programs and initiatives
  • Providing key information to rural health care providers
  • Encouraging collaboration among partners as the most effective way to make accessible, quality health care a reality for rural populations
