Community-Based Workforce for COVID-19 Vaccine Outreach

SITUATION: Since its first emergence in 2019, COVID-19 has severely impacted Northeast Florida and disproportionately affected racial/ethnic minority groups and individuals living in areas of high social vulnerability.

PROJECT OBJECTIVE: HPCNEF joined state local health councils in a campaign to establish, expand, and sustain a public health workforce to prevent, prepare for, and respond to COVID-19. Our focus is on directing resources to the most vulnerable and medically underserved communities within HPCNEF’s seven counties, ensuring that challenges such as low vaccine confidence and lack of vaccine accessibility are addressed. The state-wide campaign is called Stay Healthy Sunshine.

IMPACT:

  • Mobilizes local Community Outreach Workers (CHWs) and Community Health Advocates (CHAs), including patient navigators and social support specialists
  • Conducts face-to-face outreach with community members at public events
  • Making vaccine appointments
  • Provides resources to find convenient vaccine locations
  • Assists individuals to schedule transportation or overcome other barriers to get to vaccination sites
  • Monitors and tracks number of vaccine appointments and vaccinations
