As we enter the Post-Public Health Emergency phase of the pandemic, the partners of the Advancing Health Literacy program want to hear how COVID-19 currently affects our Northeast Florida community members.
Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia County residents are asked to take this 4-minute survey to share their thoughts. Collected data is kept anonymous.
