The Florida Department of Health in St. Johns County has partnered with us to launch its community survey today. St. Johns County residents 18 years and older are encouraged to fill out a 10-minute survey to share their opinions about healthcare and the quality of life in their county. These survey results, along with feedback from key stakeholder interviews and focus groups, will help guide a detailed Community Health Improvement Plan (CHIP).
Please help us by sharing the survey with friends, family, coworkers, community partners, and neighbors. The survey is available in both English and Spanish. All responses are confidential, and while some demographic information from the survey will help us identify any particular groups that need specific services, any responses are completely anonymous. It will be open through Tuesday, April 30, 2024.
