PPI Project

SITUATION: Infectious disease-related health disparities and health inequities continue to pose problems for citizens of Jacksonville, FL, especially for high-risk and underserved populations.

PROJECT OBJECTIVE: HPCNEF works with the Florida Department of Health in Duval County and local partners with funding from the U.S. Department of Health and Human Services through the State Department of Health to provide services that promote, protect, and improve health for high-risk, low-trust Jacksonville populations that face barriers to accessing health services. As project lead, HPCNEF partners with community providers and organizations to address minority populations through education, support, and health information. This grant is part of the Centers for Disease Control and Prevention’s (CDC) National Initiative (OT21).

IMPACT: 

Provides services aimed at outreach and education to the community at large with the goals of:

  • Reducing health disparities
  • Improving the quality of trustworthy health information
  • Increasing distribution of equity-enhancing information related to health and health conditions to disproportionately affected populations
