We work to improve population health throughout all communities in Northeast Florida by dedicating resources and expertise to health planning and research, health promotion and education, health data management and analysis.

We are the local health council that represents Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia counties.

Why We Do It

We believe that health connects all populations and their communities, and we strive to protect and improve health outcomes of all people.

How We Do It

Everything we do is aligned with the conceptual framework developed by the Department and Health and Human Resources (HHS) and the Center of Disease Control (CDC) known as the Social Determinants of Health (SDOH). SDOH are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. SDOHs respond to changing priorities in communities and public health.
